Conditions:
1. All applicants must fill out full name and information requested in the application.
2. In case we find out that you do not tell us the truth on your application, we will terminate your rights without advance notice.
3. Each applicant can only apply once.
4. To receive our special offers and benefits, we need your full name and patient identification number (HN).
5. We have the rights to terminate an agreement with out giving a notice if we see it suitable.
6. Your information will be kept secret.
7. We may e-mail you information as needed.
